EntireoPathic Medicine™ proposes that health and disease emerge from interacting biological, psychological, behavioral, social, environmental and other patient-specific determinants.

Many individual components of this proposition are supported by substantial bodies of modern scientific research.

Scientific precision, however, requires an important distinction:

Evidence supporting the individual principles incorporated into EntireoPathic Medicine™ should not automatically be interpreted as clinical validation of EntireoPathic Medicine™ itself as a unified therapeutic system.

Direct evaluation of the integrated EntireoPathic framework requires dedicated research.

We consider that distinction essential to responsible scientific communication.

The Biopsychosocial Model

George L. Engel’s landmark 1977 article in Science, “The Need for a New Medical Model: A Challenge for Biomedicine,” challenged strictly reductionistic models of disease and proposed consideration of biological, psychological and social dimensions.

The biopsychosocial model became highly influential in modern healthcare.

Social Determinants Of Health

Modern public-health research demonstrates that health is profoundly influenced by conditions in which people are born, grow, live, work and age.

Income, education, housing, food access, transportation, neighborhood conditions and social resources may influence morbidity, mortality and access to healthcare.

The World Health Organization and major public-health institutions recognize social determinants as fundamental contributors to health.

Social Connection And Mortality

Social relationships themselves may influence health outcomes.

A widely cited meta-analysis involving 148 studies and more than 300,000 participants found stronger social relationships associated with increased likelihood of survival.

Stress And Physiology

Chronic psychological stress can influence multiple physiological systems.

Research on allostatic load describes how repeated or chronic stress responses may affect neuroendocrine, cardiovascular, metabolic and immune processes.

Psychoneuroimmunology

Psychoneuroimmunology investigates communication among psychological processes, the nervous system, endocrine pathways and immune function.

This scientific field provides important evidence that traditionally separated dimensions of human health interact biologically.

Lifestyle And Chronic Disease

Tobacco exposure, diet, physical inactivity, alcohol consumption, sleep and other behaviors contribute substantially to preventable disease.

Preventive medicine therefore requires attention to behavior in addition to diagnosis and pharmacological treatment.

Lifestyle interventions are not alternatives to medically necessary therapy.

They are frequently components of evidence-based medical care.

Physical Activity

Extensive research associates regular physical activity with reduced risk of cardiovascular disease, type 2 diabetes, certain cancers, functional decline and premature mortality.

Exercise also contributes to physical function and psychological well-being.

Sleep

Sleep influences metabolic regulation, cognition, cardiovascular function, immune function, mental health and overall health.

Recognition of sleep as a health determinant illustrates why comprehensive medical assessment may require examination of factors extending beyond the patient’s principal diagnosis.

Nutrition

Dietary patterns influence cardiovascular risk, metabolic disease and other health outcomes.

Modern nutritional epidemiology and clinical research continue to investigate relationships among dietary patterns, metabolism, inflammation, microbiota and chronic disease.

Placebo, Context and the Clinical Encounter

Clinical outcomes can also be influenced by expectations, conditioning, communication and contextual aspects of treatment.

The scientific study of placebo and nocebo effects demonstrates that the clinical encounter itself can produce measurable physiological and subjective effects.

This does not mean that disease is imaginary.

It demonstrates that human physiology and human experience interact.

Patient-centered Care

Patient-centered care emphasizes individual needs, preferences, values and participation in healthcare decisions.

Modern healthcare quality frameworks increasingly recognize patient experience and shared decision-making as important components of high-quality care.

Spirituality And Health

Spirituality and religion have been investigated in relation to coping, quality of life, medical decision-making and patient experience, particularly in serious illness and palliative care.

This literature requires careful interpretation because associations do not necessarily establish causality.

Nevertheless, spirituality may be clinically relevant when it matters to the individual patient.

Palliative Care As a Whole-person Model

Palliative care provides an important example of multidisciplinary healthcare addressing physical symptoms alongside psychological, social and spiritual concerns.

In a landmark randomized study involving patients with metastatic non-small-cell lung cancer, early palliative care was associated with

improved quality of life and mood and, in that study population, longer median survival.

Integrated Care

Increasing recognition of multimorbidity has demonstrated limitations of treating chronic diseases entirely in isolation.

Patients with multiple chronic conditions may receive recommendations from numerous specialists, creating problems involving medication burden, conflicting goals, fragmented communication and competing treatment priorities.

Whole-person care seeks to integrate these considerations.
